Farada Mental Health Consent & Fee Agreement
This fee agreement outlines the financial terms and conditions of our services to ensure clarity and transparency regarding fees, payment methods, and cancellation policies. Please read through the agreement carefully and feel free to ask any questions or express any concerns with your clinician before signing this document.
OUT OF POCKET FEES (Not Using Insurance):
Taiwo’s fees:
$275 for 55 minute sessions.
$245 for 45 min sessions.
$215 for 30 minute sessions.
$175 for 15 minute sessions.
** These fees are subject to increase over time at the practice owner’s discretion. You will be informed well ahead of time of his change were to occur. **
Sliding scale fees are available for clients who have expressed financial difficulties and do not have/are not using insurance. This agreement will be reevaluated every 6 months to 1 year. If you wish to explore this option, please speak with your clinician. We will do our best to accommodate your needs!
INSURANCE: I am primarily an out of network practice, except for certain plans with Cigna and United Healthcare. Please note that the client will be responsible for the co-pay, co-insurance and or deductible amounts as per their insurance plan’s rules. Insurance will be billed for all services covered by your insurance carrier. It is understood that if your plan does not cover some services then you will be responsible for the full cost, and it will be based on the out of networks rates as stated below. We will inform you which services will be covered before services begin.
It is your responsibility to inform us if your insurance changes. You will be responsible for all services rendered during a lapse in coverage or if for any reason we are unable to bill your insurance company. Please note that in network rates are based off your primary insurance carrier. If you have secondary insurance, we may submit this on your behalf, though we will only base network rates off your primary carrier.
Co-pays/Deductibles/Co-Insurance will be billed to the card on file. Your insurance company enforces these numbers, and we are contractually obligated to honor all rates processed through your insurance company and charge you according to their rules. CREDIT CARD SURCHARGE & ZELLE PAYMENT: Farada Mental Health offers the ability to pay by a Credit card, FSA, or Debit card. Stripe charges a 2.9% plus $0.30 fee per transaction for online card and wallet payments, with additional fees for international cards and currency conversions. Thus, all clients are charged an additional fee for credit card processing.
To avoid the the additional processing fee, clients can pay their bills via Zelle. Zelle payments can be sent to Taiwo.m@farada-mental-health.com
LATE CANCELLATIONS: If you are unable to keep a scheduled appointment, please notify our your clinician at least 48 hours in advance. Unforeseen emergency situations will be taken into consideration. Please be aware that we cannot bill your insurance company for missed appointments or appointments cancelled without 48-hour notice. In these situations, you will be responsible for your entire fee.
NON-PAYMENT: If you are having financial issues that prevent you from paying your sessions on time, please speak with your clinician and collaboratively discuss and implement the best options for you moving forward. We understand life happens sometimes, and this can unfortunately impact your financial stability, which is why communication is essential in these situations. We are here to help you to the best of our ability and are willing to work with you.
